Open Office Hours for Vermont Public High Schools to learn more about P2P:

What is Pathways to Partnerships (P2P) project?

Brings together Vermont nonprofit organizations to support students with disabilities, ages 10–24 who are on an IEP or 504 plan, as they transition from high school to adult life.

How does it impact change for students with disabilities?

Through student-centered planning, self-advocacy, peer mentoring, and work-based learning, the project works alongside students, families, and school professionals to strengthen transition planning.

What’s the benefit to Vermont schools?

P2P partnered schools get full access to Pathful (formerly Virtual Job Coach). Pathful can be used for transition assessments as well as career exploration. Schools also have access to professional development around transition planning.

How can students benefit?

Middle school and high school students can access Pathful .

Who are the community partners schools could collaborate with?

Partners include the Vermont Center for Independent Living, VT Agency of Education, HireAbility, Green Mountain Self-Advocates, Vermont Family Network, Vermont Student Assistance Corporation, the Center on Disability and Community Inclusion, Vermont Developmental Disabilities Council, among others,

Our Invitation:

To help introduce the project, we are hosting virtual Open Office Hours in April, focused on new schools. All schools are welcome to drop in any time during the hour. If your school already partners with P2P, we look forward to continuing this work and welcome your questions or suggestions for the year ahead.

  • April 27, 3:30–4:30
